Registration begins with the completion and submission of the following documents. Please choose the packet that corresponds to your Laboratory space and Biosafety Level request:
Level 1 Registration Packet - (Registration Application, User Agreement, and User Agreement Acknowledgement)
Level 2 Registration Packet - (Registration Application, User Agreement, User Agreement Acknowledgement, BSL 2 Checklist)
Level 3 Registration Packet - (Registration Application, User Agreement, User Agreement Acknowledgement, BSL 3 Checklist)
The Registration forms included in each packet are also available individually below:
IRR Registration Application - this form is for initial registration only; do not place orders on this form.
User Agreement - signed by a duly authorized official at your institution.
IRR User Agreement Acnowledgement - signed by the registrant.
Biosafety Checklist (use the form below that applies to your Laboratory space)
Each IRR registrant institution is required to sign a User Agreement (UA) with the ATCC®. The institution is responsible for ensuring all IRR registrants associated with the institution are aware of, and accept the terms of the UA. The UA is an agreement that governs the transfer of research materials between two organizations. The UA is not intended to limit the distribution of biological materials for non-commercial research purposes. The UA program for use of IRR products in research applications protects the ability of the CDC and ATCC® to supply free reagents for the benefit of the public health community. The document defines the duties of the recipients and their institutions, including the responsibility to properly handle, use, and dispose of the materials according to applicable laws and regulations.
Your IRR Registration Application and the scope of use you provide on the IRR Application will determine the reagents you can access. Please make sure your scope of use includes all reagent types which will be requested.
Once these forms are completed, it can be e-mailed to contact@influenzareagentresource.org or mailed to the following address:
